I authorize the laboratories, facilities, physicians, nurses and technicians contracted by Gray & Son, Inc. and Maryland Paving Inc. to collect the necessary specimens for the purpose of determining the presence or absence of controlled substances. I furthermore authorize the disclosure of the results of such testing only to the appropriate company officials. I understand that Gray & Son, Inc. and Maryland Paving Inc. are responsible for maintaining the confidentiality of all testing results.
In connection with my application for employment (including contract for services or volunteer services), consumer reports will be requested. These consumer reports may include the following types of information: names and dates of previous employers, salary, work experience, education, accidents, licensure, credit (except California), etc. I further understand that such reports may contain public record information such as, but not limited to: my driving record, workers’ compensation claims, judgments, bankruptcy proceedings, criminal records, etc., from federal, state and other agencies which maintain such records. In addition, investigative consumer reports as defined by the federal Fair Credit Reporting Act, gathered from personal interviews with former employers and other past or current associates of mine to gather information regarding my work performance, character, general reputation and personal characteristics, may be obtained.
I expressly authorize, without reservation, the employer, its representatives, employees or agents to contact and obtain information from all references (personal and professional), employers, public agencies, licensing authorities and educational institutions and to otherwise verify the accuracy of all information provided by me in this application, resume or job interview. I hereby waive any and all rights and claims I may have regarding the employer, its agents, employees or representatives, for seeking, gathering and using truthful and non-defamatory information, in a lawful manner, in the employment process and all other persons, corporations or organizations for furnishing such information about me.

Additionally, nothing contained in this employment application and anything conveyed during an interview is intended to create an employment contract, implicit or implied. I also understand and agree that any future changes in duties, compensation, working conditions, benefits or company policies and procedures will not alter this at-will arrangement.
If I am applying for a position as a CDL or Non-CDL driver, I am required to possess a current and valid driver’s license at all times. I understand that any omission, misstatement or inclusion of false information on this application or any document used to secure employment with Gray & Son, Inc. or Maryland Paving Inc. shall be grounds for rejection of this application or discharge if I am employed regardless of the time elapsed before discovery.
I understand that this application remains current for only 60 days. At the conclusion of that time, if I have not heard from the employer and still wish to be considered for employment, it will be necessary for me to reapply and fill out a new application.
I also understand that if I am hired, I will be required to provide proof of identity and legal authorization to work in the United States and that federal immigration laws require me to complete an I-9 Form in this regard.
